Before there were anesthetics, surgeons had to work as fast as possible. Their patients were wide awake and fully aware of their pain. At best, the person being cut open might get a strong alcoholic drink to make them dizzy. In some cases, a blow to the head could knock them out long enough to get the job done without a struggle.
